Thank you for taking part in our pilot for online submission of statutory notifications of the death of a person using your service (SN16).

The online notifications pilot has provided us with useful insight about online notifications, and has helped us to develop a more sophisticated online notifications tool, which we have recently launched on the CQC Provider Portal.

The new online notifications tool allows Provider Portal users to submit statutory notifications SN16 (death of a person who uses the service) and SN18 (other incidents) via easy to use, responsive online forms.

What will happen next?
During the next few weeks, we will send you an email invitation to create a Provider Portal account. Creating an account is a swift and straightforward process; all instructions will be set out in the invitation email.

As well as submitting statutory notifications SN16 and SN18, you will also be able to use the Provider Portal to apply for variations to your registration.
Once you can access the Provider Portal, you should no longer use the online notifications pilot to submit statutory notifications SN16 (death of a person who uses the service). Instead, these notifications should be submitted via the Provider Portal.

From 12 June, access to the online notifications pilot platform will be restricted. You will still be able to log in and view your previous submissions, but you won’t be able to submit any new forms.
